| Lee R***** | Nurses are friendly and patience. Take extra care of those who most needed. Reassuring presence when required. Able to provide professional advice. A pleasant place for treatment. |

| S*** | Although my stay at Diaverum Fajar was short, it was a place I can call "home". The staff there are very friendly, close-knitted, and function more like family members than anything else. Jovialness is always in the air, which made their professionalism meaningful and memorable. I had to move out to another dialysis centre because of insurance-related issues. Before l left, I gave feedback that Diaverum should register itself as a partner to my insurance provider, so that I could come back to Diaverum. For making my stay so memorable, my special thanks goes to : - Shafiqah - Shahidah - Fathima - Tracy - Eileen - Grace - Ayu - Richard I am sorry if I have left out any other name. And lastly, I must say a thousand thanks to Sherlyn for making my stay at Diaverum beaucracy-free. Thank you so much. |
| N******** Suparman | The friendly and family like atmosphere here at Fajar has made the pain of having to undergo dialysis, bearable. They will always find the time to ask about your wellbeing or just be there if you ever need a listening ear. Always doing their best to accommodate to your requests. |

To ensure you receive proper care and treatment, we would require you to bring the following, where applicable:
- Referral form (enclosed, to be endorsed by renal medical officer and above), if any
- Dialysis prescription
- Dialysis flowsheet of last 3 sessions
- Recent blood tests with serological results (e.g. for Hepatitis B)
- Other relevant medical information (e.g. discharge, medication list etc)
Remember your kidneys used to clean your blood 24 hours a day, 7 days a week! When you skip treatments, extra fluid will need to be removed when you go back for dialysis and this may make your next treatment harder on yourself.
If you feel unwell on your dialysis day and feel unfit for treatment, you should seek for immediate medical attention and make a call to the clinic to inform them with regards to your condition.
If you have a planned medical appointment outside of the clinic and you need to change your dialysis treatment time for that day, you should let the clinic Head Nurse know in advance and give them as much notice as possible. We strongly advise that you should arrange all medical appointments on a non-dialysis day.
Wear comfortable clothes that allow easy entry to your access site (e.g. Short sleeve tops). You can also bring a blanket for your own use during treatment as it may get cold.
